Rottie?s melanoma treatable, but not curable
Question: My dog has just been diagnosed with a melanoma that is growing from his toe. I recently noticed a black hard lump where his nail grows. When my vet examined my dog, a big lymph node was found. A test was done and my vet says there are cancer cells in the lymph node, too. I am being told that the prognosis for my dog is terrible. He is an 11-year-old Rottweiler. Is there anything you can do?
Answer: I am glad to say that there are treatment options for your dog. Unfortunately, melanoma is a very aggressive cancer and once this cancer finds its way into the lymph node, it is not usually curable. I am guessing that you were given a terrible prognosis for your dog because this is not a curable cancer.
Although it is true that lymph node positive melanomas are not usually cured in canine patients, the way that we look at cancer in our geriatric patients is somewhat different than the way we may look at cancer in a young or middle-aged dog. In older patients, we do not have to eradicate every single cancer cell in the body for a patient to be able to live with his disease. We just need to maintain a low enough level of cancer to prevent cancer-related symptoms or discomfort.
For example, at the current time, your dog is probably uncomfortable since there is a tumor growing from his nail bed. The first treatment step will be to alleviate this discomfort by removing the tumor. To decrease the amount of cancer in your dog’s body, the next step will be to remove the lymph node that has cancer cells present in it. After that, we will want to prevent the cancer cells from travelling from the lymph node to the lung by using a treatment called immunotherapy. This uses your dog’s own immune system to fight the cancer that is in his body. Most patients have very few side effects from this type of cancer therapy.
By treating your dog with a combination of surgery and immunotherapy, there is a good possibility we will be able to extend your dog’s life and quality of life. By treating cancer as a long-term disease, and attempting to maintain a low level or stable amount of cancer in the body, your dog can live out the remainder of his natural life comfortably with minimal symptoms of his cancer, and minimal side effects from the treatment. As we discussed earlier, this is probably not going to be a cancer that we cure, but one that we can manage for the remainder of your dog’s life. I wish you well.

Distemper extremely contagious, worse for puppies
Question: My neighbor got a new puppy and the puppy came over and played with our dog. It was a very cute puppy but it soon got sick and was sneezing. They brought it to a vet and it was given antibiotics for kennel cough. A couple weeks later it started to have seizures and the vet said it had distemper. They had to put it to sleep! My dog has been around this puppy a lot. Is my dog going to get sick? –Naomi
Answer: I am sorry to hear about your friend?s puppy. Distemper is a viral infection that can result in severe symptoms including death. The virus first attacks the respiratory cells and the dog has a runny nose with sneezing and coughing.
It is often impossible to tell the difference between distemper and the many other viruses and bacteria that can cause ?kennel cough?. Many dogs will seem to improve but the distemper virus is marching deeper. It attacks all the epithelial cells in the body. The dog may then get pneumonia. The nose and feet may grow thick hard calluses. The dog may have trouble swallowing or eating and can spike a high fever.
As the virus continues to replicate it will reach the brain and nervous tissue. The dog may develop seizures. Seizures may become almost constant and severe.
Dogs can shed virus for weeks to months. The distemper virus in an enveloped virus and is easily killed in the environment. It dies rapidly once outside of the body. Droplets or ?nose to nose? contact generally transmits it. Simple cleaning will kill the virus.
Vaccination is extremely effective at preventing infection. If your dog is up to date on his vaccinations, he should stay healthy. If he has not been vaccinated, he is in great danger of getting distemper infection. It is best to get a distemper vaccine booster as soon as possible. Since your dog was exposed to distemper, it may be best to have him vaccinated with a recombinant distemper vaccine. The advantage of this vaccine is that it will not interfere with testing for an active distemper infection.
While most puppies die from distemper virus, adult dogs have a chance at survival.
Adult dogs have a better immune system and can sometimes rally enough antibodies to fight off the infection in time.
It is always best to quarantine a new dog or puppy for two weeks before introducing them to other dogs. New puppies can be incubating a number of bacterial and viral illnesses. Make sure all puppies and dogs are up to date on vaccines. Many of the worst infections are preventable by vaccination.

Is recovery from lymphoma possible?
Question: My dog, Girlie, is 12 and I just found out she has cutaneous lymphoma. It started as just itchy patches that she aggravated and infected. Then she was on meds for two weeks, and now, in the third week, a big lump is hanging in her groin area. There are nodes running down her leg. She is not in pain, but how does it spread so fast? My vet suggests I put her to sleep. I feel he is right, but is there ever recovery from this type of cancer in older dogs? – Joanne
Answer: From your description of Girlie, it sounds as if she has cutaneous lymphosarcoma with lymph node involvement. Lymphosarcoma is cancer of lymph nodes and lymphoid glands. When the disease is present in the skin, we call it cutaneous or skin lymphosarcoma.
This cancer can begin as you have described with patches of itchy, scaly skin.
The disease can progress quickly and lymph nodes in the groin can become enlarged. There is treatment available for this disease, but this type of cancer is usually not cured with treatment.
The aim of treatment is to provide comfort to Girlie by reducing the size of the lymph node swelling and by eliminating the itchy areas in the skin. The treatment for cutaneous lymphosarcoma is usually oral chemotherapy, which is very well tolerated even in older patients. Most oral chemotherapy protocols do not cause a poor appetite or gastrointestinal upset.
Most patients that receive oral chemotherapy continue to have a very good quality of life, with very minimal side effects to treatment. There is about a 50 percent chance the treatment will work for Girlie and she will be more comfortable. If the treatment works, it can work for six months to a year and possibly longer.
I recommend you have Girlie evaluated further so her disease can be staged and we can determine how progressed it is. Once we have more information, we can work out the best treatment plan for her.

Dachshund needs training to stay off bed
Question: I have a 4-year-old dachshund named Jake, and I am trying to train him to stay off the bed. Can you give me some ideas on how to manage this? My boyfriend and I are at wits? end. He whines when I leave him downstairs gated, also I have tried to close the door, but that doesn?t work. – Gale
Answer: This is a common problem. Teaching a dog to be relaxed away from the owner is so important, yet it is often overlooked in puppyhood.
Instead of putting him downstairs, you can simply let him sleep in your room. Move the stairs or furniture that he uses to get onto your bed, give him a comfy place to lie down in your room and go to sleep. He will whine in this situation as well, however, it is best for all if you ignore him. If he can launch himself up onto the bed without any help (that is one athletic dachshund!) then, you will have to put him back on the floor gently each time that he jumps up on the bed.
You can also confine him downstairs with a cushy bed and accomplish the same thing. Make sure to leave him with a great food toy or chewy that he never gets at any other time. This will keep him occupied for a couple of minutes before bedtime.
Most dogs who are not used to this type of thing will cry in the beginning. If the dog is completely ignored, most often the crying will stop in about three nights. Completely ignoring your dog means you can’t make eye contact, he can’t see you and you can’t talk (or yell at) him. Just put your earplugs in and give it three nights.

Limping dog needs at-home exam
Question: My dog started limping recently and I am not sure if he bumped something or if he fell. He seems to have the most trouble in the morning when he gets up. Could it be because of his age? Is there a supplement i can give him? ? John
Answer: You need to do a mini home exam on your dog first. Be sure to pick up his feet and check between the pads for stickers or thorns. If he has matted hair between his pads this can cause discomfort. Trim out the hair between the pads.
Make sure his nails are trimmed short. Toenails can sometimes grow so long that it can cause a limp or even grow into the pad. Check to make sure none of the nails are broken or split. Look for cuts or sores on the pads. The pads should look smooth, like leather. Check for attached ticks between the pads and between the toes.
Look for scratches, cuts, or swollen areas on the legs. Older dogs are more likely to have arthritis or degeneration of the joints, but even young dogs can have this condition. There are special diets like Hills Prescription diet J/d for joint health. There are many supplements and prescription medications for arthritis. Ask your veterinarian to suggest a good one for your dog.
If you didn?t find anything on your mini home exam and your dog is still limping, he will need an examination by your veterinarian.

Smaller dogs often more difficult to housetrain
Question: We have two dogs, and my husband and I both work. One year ago, we adopted a third dog, because our friend could no longer afford the dog. She is a small poodle and is very affectionate.
Unfortunately, we did not know she is not housetrained. She will go on the tile floor or on the leather couch. We walk her or let her out in our back yard in the morning before work, in the afternoon when the children are home after school, and many times after 4 p.m. When we are not home, she is in a pet cage, which she likes. She is 3 years old. Is it to late to train her? We love her, but the situation has to be corrected. What should we do? – Sandra
Answer: Smaller dogs often have a greater likelihood of housetraining problems. It is unclear why this happens, possibly because many dogs are trained on pee pads. Those can confuse them and lead to accidents when they are introduced to a home where they are expected to urinate and defecate outside.
Some dogs aren’t well socialized and are reluctant to eliminate outside due to fear. Either way, it is not too late to re-housetrain your dog. You can find information at this link.
